Stroke Co-Ordinators (Community)

Community Stroke Specialists

We are a small multi-disciplinary team who follow up all stroke patients for 12 months post-stroke. We review patients at home within 6 weeks of discharge and at 3, 6 and 12 months. We will review patient’s recovery, ensure that they are receiving the correct support, review and address their risk factors and give advice and information (e.g. healthy lifestyles; driving; return to work/leisure activities). We will signpost and refer to other services as necessary. We are the patient’s “single point of contact” for their post-stroke journey. To access our service a referral is required from a healthcare professional following a confirmed diagnosis of a recent stroke by a doctor or consultant.

We are based at West Park Hospital and are commissioned to see Wolverhampton and Seisdon peninsula patients in their own homes.

Opening / Visiting Times

The service is run Monday to Friday 8.00am - 4.00pm but the office is not staffed.
