Repairs to Orthoses

Please keep your orthoses in good repair. If the orthosis you have been provided with breaks or becomes worn contact the department regarding its repair. For example, if you have received footwear you can drop them into the department to be re-soled as necessary, or to have Velcro straps replaced on splints or new covers on functional foot orthoses/insoles. Please ensure you label your devices when dropping them off. We will contact you when they are ready for collection. Please note, no orders can be placed for patients who have not been reviewed by the orthotics service within the last 12 months.

In order to gain feedback from our service users, the Royal Wolverhampton NHS Trust uses The Friends and Family Test (FFT).

The Friends and Family Test is a feedback tool that supports the fundamental principle that people who use NHS services should have the opportunity to provide feedback on their experience. Feedback is extremely valuable and counts in shaping our future, helping us to meet our vision and values and helps us to identify any possible areas where our services could be improved.

Footwear Adaptation Agreement Form (PDF, 91Kb)

This form must be signed by the patient/guardian for EVERY footwear adaptation carried out on the patient’s own footwear (not footwear supplied by the NHS).
